General educational information, not medical advice. Discuss individual health decisions with your treating clinician. Bjou service availability must be confirmed separately.
Work with the discharge team
The treating team determines discharge readiness and clinical follow-up. Families can discuss practical questions such as transportation, entry access and who will be available at home.
Identify separate support needs
A ride home, nonmedical assistance, nursing visits and equipment may involve different providers and requirements. Clarify which services are actually confirmed and who is responsible for each handoff.
Keep information in the right channel
Public inquiry forms are not a place for discharge records or detailed medical histories. Ask the team to establish an appropriate secure process when sensitive information is necessary.
